Recurring torn file icon in Organizer after saving in Editor with caption 'editing in progress'

I have a recurring problem after saving edited images in Editor. After I edit then 'save as' the original Canon CR2 raw file as either a psd or jpg file on an external hard disk. However after I close the file in Editor, I get a torn file icon back in Organizer with a caption 'editing under progress'. This is an irregular event but I get this often enough to be a real pain.

What am I doing wrong? Thanks for your help. Sergio
