Broken lines on horizontal in video

I am new to Premiere Pro, I am getting a brake up of horizontal lines as can be seen in the picture attached. I have tried changing size, output etc. Original video is an mp4 and when played back it does not have this brake up of the horizontal lines. Please what am I doing wrong?

What is the pixel size of the original and what are the sequence settings?
