Encore CS5 button problem

I have created an Encore menu in Photoshop. Only some of the buttons work in Encore. I am sure my layer naming is correct. Pls see attached - can anyone tell me why button 3 is OK but button 4 is not? Button 4 appears to be recognised as a layer (?)

=x designates one of 3 colors groups for button highlights. There is no =4. Make them all =1. You use =2 or =3 when you want more than one color in a highlights.
