While attempting to reduce an audio track's volume, I noticed chanegs to the audio gain (via the context menu) or audio levels (via the effects panel) had no effect on the audio level.

Edit: the clip is remuxed from an mkv file to an mp4 file via OBS Studio. The remuxing was successfull and no error messages were displayed.
Try resetting the preferences:

If that doesn't work, try resetting the Workspaces:
Reset a workspace
Reset the current workspace to return to its original, saved layout of panels.
1. Do one of the following:
• Click the Workspace menu icon and select Reset to Saved Layout.
• Choose Window > Workspace > Reset to Saved Layout.

Resetting the window layout revealed there was another audio track that was causing this mess. This issue has been resolved.
