Playing back a sequence on PP that's 4k (4096) is fine, but when the setting is at UHD (3840), the image looks squashed (too wide). Output via Blackmagic Decklink HD extreme to Flanders monitor. FSI monitor says 2K...not sure why that is?

Lot's of variables here but your best bet is to go through your entire signal chain and check everything is set to UHD (3840x2160, square pixels).
Check your premiere sequence settings and Decklink output settings. If the Flanders is reporting 2k then the BM card is probably trying to downconvert.

I understand that you are getting incorrect resolution while playing a media file in Adobe Premiere Pro.
As Mike suggested, we need to check the Pixel Aspect Ratio in Sequence Settings. Please share a screenshot of the sequence settings.

Sorry for the late reply. Sequence is set to square pixels. The BMD card is set to downconvert.
