Do I need a photo with the model holding the release or what else can be used for visual reference?

 

I am looking for the simplest solution here.

An identifiable image of the model is required.

Yes, the face needs to be visible. If you have a picture with different models, you need as many releases than models. And in a way or another, you need to make it clear whose release it is. It's for a visual reference, so that Adobe can attribute the release to a certain person (or with a property release: property). There is no other reason for this requirement.
