The process of working with Frame has been almost identical since version 7 for most of the features. There were some major interface changes along the way (panels rather than dialog boxes) but the overall function remained the same. Obviously there are new features, but the majority of them affect structured documents.
For you book numbering, just right-click as I mentioned.
The TOC and Index are formated on the reference pages. You can use the current help file as the process is the same.
Your Index is not a generated file--you must have added it like a regular chapter. Note the color of the TOC in the book; all generated files must have that color to update when you update the book.
To get your existing index to work with your book, remove the index from the book and add the Index in the same way you did the TOC. It should be listed in the menu just below the TOC option.
If you have an existing index design, before you add the generated index--rename a copy of your "good" index the same as your current one. The pattern is simple: the Book file name with the generated code (TOC, LOF, LOT, IX, etc.). Be sure to create it with the menu--do NOT add it like a regular file.
