why is audio on track 1 and II mono and the audio for track 3+ is not playing at all...
It sounds like you might be using a Broadcast sequence preset, which routes audio tracks to different channels rather than mixing them to a stereo output.
To fix this, select and drag your clips from the current sequence to the New Item button (document icon) at the bottom right of the Project panel. This will create a new sequence with a stereo mix.

I did not set a preset nor change any of the preferences when the new version automatically updated on my machine. The new version has fallen flat and it can't get back up very easily! Ungh!

I did go back and look at the preset that was used and it was not my preselected preset of 29.97 fps and 1080p - Premier Pro chose one and I was not aware of what it had done - Now I know what to do! Thanks!

I performed this solution of dragging all clips to the "New Item" button and the sequence it created was a 1080p sequence and not the 4K Sequence which I was editing in. Any other solutions for this? Is there a way to make the "New Item Sequence" the same?

The New Item option will take the framesize/rate from the first selected clip. Note, this is as it sees the 'first selected clip' which may not be what you expect at times.
Sometimes it's best to pick the clip with the audio tracks & framesize/rate you need, create a sequence for it, delete the clip, then move the clips you need in the order you need.

Is there a way to change this without making a new sequence? It's playing tracks one and 2 as stereo (they are dialog) and then tracks 3 and 4 (music) make no sound even though I see levels bouncing.
This has basically become the default audio config for when I open a project in premiere 2025 and I can't figure out how to change a sequence to just output in stereo. Not sure why anyone would want this?

You can change your sequence to stereo by changing the number next to the Mix track to 2, but you'll still need to re-route each track to 1 + 2 in the Track Mixer. Also, these are Mono tracks, so if you add stereo media to them, it will be ignoring the right channel.
It will be a much quicker process to add it to a new sequence.

Check your Audio Track Mixer Panel and make sure the balance dial is set to 0.0. Also check what channels the output is going to. Also, with the clip selected, check Clip->Modify->Audio Channels to make sure you have L and R channels selected.
The recent upgrade changed some intial settings. I am still working through othe audio issues since the upgrade.

Those diagonal lines on the clip means your media is offline.

Please add a screengrab of your timeline panel. And a better description. I've no idea what's going on, how can anyone help?
