Video file showing only as audio

Hi, 

I am trying to edit a video with a .MP4 file. The file only shows as audio when adding to to Premiere Pro. My Mackbook pro was updated to the latest update but I did the "MacOS Big Sur 11.4" update as I thought this would fix the problem according to research.

I have restarted the Macbook, the apps, and also cleared the Media Cache, but it is still showing as audio only. Premiere Pro is also up to date.

I would appreciate your help on this,

When importing into Premiere or inserting into the timeline?

If it's the latter:

Most common user error is not source patching the track.
Solved by first opening clip in Source Monitor, then drag/insert into timeline.

is the mp4 playing properly in the source monitor?  Does it play in other software like vlc
