Headings converted to inlineshape instead of text (MS Word 2010 to PDF)

When I create an accessible PDF in MS Word 2010 using the Acrobat plugin, some heading styles are converted to inlineshape instead of text. How can I convert them to real text? OCR doesn't work.

I am not sure I understand. Is the heading text converted to an image? If so, that seems very strange. Or, in the Tags Pane are you seeing tags that read <Inlineshape> instead of, for example, <H2>? Or something else?
