PDF Creation / JAVA Error (LOCKED)

I have created a PDF form which is part Customer Pedigree Information (i.e. Name, Phone Number, Address, etc) and part Analysis (i.e. multiple cells which include simple addition and division (with% outputs) via Simplified Field Notation Script). All of the calculations fields are working properly, however, when I attempt to input basic customer data, a Java Script Error Window pops up displaying the following message “The value entered doesn’t match the format field “X”. X represents all of the fields which are formatted to output a %. I then will have to click through several of these error messages before the desired customer input shows up onthe field (example: client’s name).  Any help with this issue would be much appreciated.
