Is it possible to allow an open ended response and have a multiple choice quiz on the same slide?

Is it possible to allow an open ended response and have a multiple choice quiz on the same slide?  We are prompting the user to select all items that apply to them (via the multiple choice option - with multiple answers). AND we would like to add an other category and allow free-form answers. Is this possible?

    Not with default question slides, could be done with custom question

    slides. Does it have to be reported to a LMS?

    Yes, the desire is to capture the information for a report in the future - i.e. this % of users choose this option and % of users said in an open-ended question this.

    Do you mean that either the answer to the MCQ or the answer to the Open question has to be scored? Is the score the same in case of a correct answer? If that is the case it could be done, using two (shape) buttons that are initially hidden. One of the buttons has a score equal to the one you want to add for a correct answer, the other a score equal to 0. With an advanced conditional action you can check the answer, and show the appropriate button. Since quizzing system variables are read only, this will result in a correct value for the Total score as well. I explained this technique a while ago in:
