Fillable PDF not fillable after download from weblink

I've made this fillable PDF, it works when i email it to people, but when i ask them to download it from this link: https://cpt.org/wp-content/uploads/test-form.pdf  its fillable qualities stop working. I've also attcahed the original.

 

Any suggestions on how to fix this? Thanks in advance

    The file you attached has fillable form fields, but the file in the link doesn't. How are you uploading it there, exactly?

    The issue is most likely with the web-server you're using. Maybe it strips the form fields...

    I am at loss too. The form is uploaded as one normally would. Good idea, i should check with the web-server folks
