Exported Media always has file name displayed

When I produce a clip in PP CS5 (Win. 10), it always has the file name as a title when played, which shows in the exported clip for about 5 seconds. I can't produce anything that doesn't have the unwanted title in the actual video, and I'd love to make it stop. I've searched and searched but have found nothing regarding this. Help??

Premiere doesn't do that. That is something your media playing app is doing.

Neil is 99% probably right in there is a 1% chance it's one other thing. In the Export Settings window, double check the Effects tab and make sure that "Name Overlay" isn't checkmarked.
