Bookmarks - Unable to add further bookmarks to a saved pdf. Option is now greyed out.

Bookmarks - Unable to add further bookmarks to a saved pdf. Option is now greyed out.  I password protected against edit, have now removed the password altogether but still unable to create further bookmarks.  The shortcut keystroke does add another bookmark but the option on the panel remains grey out. Can anyone help please?
