E-signature request and missing text

Hi I am trying to send a dodument out for signing by a client.

I successfully sign the document myself , but when i select request e-signature from someone else and go to review the document and select where they should sign, my signature disappears and some of the ticked elements and text also disappear?

what am i doing wrong? is there somethign that needs to eb selected to secur ethe document so items dont disappear? I though that the document was secure/ unchangable after I had signed it?
