Indesign auto second tabs bullet without any setting

What happens when you turn the preview switch on? 
What happens when you turn on the override indicator in the paragraph style or character style panel?
Have you checked for overrides coming from character styles or directly-applied formatting?

Have you considered making separate paragraph styles instead of defining multiple tabs in one style?
