Based on the fact that the slug lines for the .indd document are right on the trim line it appears that something was altered in the actual InDesign document. If the InDesign document was set up correctly to the trim size and you output the pdf with document bleed checked (as you have) then the slug line would actually appear below the bleed area. Without seeing the actual InDesign file it is difficult to say what needs to be fixed as the settings in your export to pdf dialogs look good. One last question. The best way to do spacing in InDesign is to use a specific leading value for within each paragraph and a space after amount (or in some cases space before) using the Character and Paragraph Panels. For 12 point type a leading of two extra points—represented as a setting as 14 point leading—in the character panel and whatever amount feels comfortable between paragraphs (I would suggest trying 1/8 inch to start). I can think of two possible reasons why this might have happened. The first is that several years ago Illustrator started showing the following dialog when opening an older file (either an .ai or .eps) with a newer version of the program: My memory of when this first came in (more than ten years ago) was that if you clicked OK without using Update the type appeared on screen both in the preview and artwork mode but when I ran some tests using CC 2017 and a fifteen year old .ai file it no longer shows even outlined text in artwork mode. It only shows an empty frame as if it had been embedded into the file. In either case the text can be "reinstated" by going through the Type Menu to Legacy Text to Update All Legacy Text. The other possible reason for this might be that if the file you're opening in Illustrator was originally saved as a pdf and the fonts needed for the file are not open or installed on the computer then a warning should come up that text will be converted to outlines. On a Mac while in the Finder  you can click on one of your ai files and type command-i to get the info window. In that Window you can click on the "Open with:" pull-down menu and choose the version of Illustrator you want the file to open with. For print work it is the general rule of thumb that Photoshop images should be imported into InDesign at 100% with a resolution of 300 ppi. I tried to duplicate your problem using (100%/300 ppi) images that had been silhouetted in Photoshop and they looked as good as when printed directly from Photoshop. I assume that you've been creating your documents with "Primary Text Frame" in your New Document dialog. This puts a text frame on your default Master page that is set for Smart Text Reflow. It can be identified by the icon that appears when it is selected on the Master page as shown in the screen shot below: Any other text frame set up on the Master will have an icon like the screen shot below and will not be automatically set for Smart Text Reflow: When you try to autoflow text in your document pages as you describe then the default Primary Text Frames would prevail. You can, however, make your text frames that are part of your layout into the Primary Text Frames by clicking on the icon shown in the second screen shot with the regular selection tool on the first of your custom text frames. The first thing you should always do when InDesign starts acting wonky is to trash the preference files. With a Finder window open and InDesign closed put your finger on the Option key (on a Mac) and with the Home Icon clicked go under the Go menu, choose library, click on Preferences and delete the highlighted items "Adobe InDesign" and "com.adobe.InDesign.plist". I just want to add a small bit to what I said above. First I agree with the other respondents that you should talk to your printer about what they need. Secondly I want to clarify about the way to indicate fold versus cut lines. I work in a very large New York agency where we deal with vendors from all over the world. We receive die lines from all types of printers and also create many of them ourselves. Over the years most of the die lines we've received from outside sources used the dashed lines for folds and solid lines for cuts. We've also received many die lines which used separate colors to define the folds and cuts. In our agency we've decided that the dashed line/solid line method allows for more instant recognition of the difference between the lines than having to check a swatch panel to see which is which. Either way is valid but your printer will let you know which they prefer. ... View more

Traditionally fold lines are made dashed and cut lines are solid for a die strike. Most of what you've done is correct. As long as you have the die line set to overprint you don't have to uncheck print layer. Then you can send the files to the printer with the die line in position. If the printer is requesting the job to be sent as a press quality pdf then send him two pdfs. One with the die line in position and one without. The only other thing I would do is save the die line from Illustrator as an .ai file rather than an .eps. It is illustrator's native format and is better handled by all versions of InDesign. When I look at the outside cover it appears to be centered on a page with equal margins left and right of the image and top and bottom. The inside cover does not appear to be set up in the same way. Because the inside has to back-up the outside exactly it is imperative that whatever size you made the document page and however it is placed on that page for the outside must be exactly the same—that is centered top and bottom and on the left and right. In the screen shot above I took your inside cover and using the control panel I was able to determine that it's overall dimension is 29.8 x 17.65. I then made my document page 31.8 x 19.65. It was then simple to again use the control panel to place the art 1 inch from the left and one inch from the top (note the reference point icon on the left of the control panel) which centers the art exactly. Doing the same for the outside cover should assure that they will line up correctly. When you do this make sure that the bleed is temporarily pulled in as this should all be done to the overall trim size. After it's centered make sure to restore the bleed. Note also that you actually need bleed on the right of the dark blue rectangle in the upper right. What he means is that the art for inside cover should be positioned on the page exactly like the front cover was because when he prints them they will be on opposite sides of the same piece of paper and need to be aligned for when they are trimmed. You have the front cover exactly centered both side to side and top and bottom. Do the same with the inside cover and you should be good to go. ... View more
