Opening PSD's in Lightroom to Photoshop where file deletes off disk

Hi, I've had this bug happen a few times. Normally I'll be opening few files at a time from lightroom>edit in>photoshop and Lightroom will save Metadata and then open the files in Photoshop. However, sometime in the Metadata saving, the file will be deleted off the disk. The PSD is just gone and I have to start from scratch or pull from a backup if I had made one in time.

Has this happened to anyone? I've been using LR for years this only became an issue this year.
