Error Code 86 :: 1 when importing csv file

It is extremely hit and miss whether the csv file will import properly.  Sometimes when I save the file from Excel it will import fine into AE but the row numbers will seem to be a bit off, in that the 0,0 cell seems to be where the 1,1 cell should be, and the rest of the time I get an import error.

I have attatcehd the error screenshots below along with me Excel export settings and the csv file.

Make sure the columns are named. Your first colums have no name. I've given them a name and now there is no error.
