Use the Quick Selection Tool to select the white background and press the delete key. You can do it in stages. A grey checker background represents transparency. Do not save it as a JPG as they can't have transparency.

Note, with the subscription model you don't "work in the cloud or online", you download the application on to your computer in the normal way and use it as with previous versions, it's just that Adobe checks periodically (I think once a month) that you've paid your subscription.

Are you sure the printer didn't use the term LPI, which stands for Lines Per Inch. 150 LPI would be the half-tone screen for printing commercial litho sheet fed on coated stock, and to produce this you need images that are around 300PPI.

‌If it's Photoshop CC2014/2015 you need to install the Creative Cloud app. Did you uninstall PS before updating your computer, if not you may need to get in touch with Adobe to reset your usage count.

And if your local print shop is using CMYK to print, you'll loose around a third of your RGB colours (whatever colour space you're using). To get the best reproduction from your paintings, use a printer, such as a high-end epson inkjet with around 10 colours. If you don't want to buy your own printer you can use a high end digital printer, for example, here's one in the UK: https://www.theprintspace.co.uk (You'll still need to calibrate your monitor and use printer profiles)

‌You may still be able to get PS CS6 directly from Adobe. Regarding the Creative Cloud applications, you don't have to be connected to use them. You download the application in the normal way, and the system automatically checks (once a month I think) that you're up-to-date with your subscription. It's very straightforward and you have quick access to updates. And, if you don't want to use the software for a bit, you can suspend payment till you need it again.

‌As your printer hasn't given you a spec it'd probably best to work in RGB in InDesign, and export the InDesign document using the Acrobat PDF Presets, selecting PDF/X-4. In the subsequent Acrobat panel, select individual pages (not spreads), and tick Use Document Bleed Settings.

Are you sure you trashed your InDesign preferences – did you see the question? Close InDesign and relaunch it, and IMMEDIATELY hold down Ctrl + Alt + Shift (Windows) or Cmd + Ctrl + Opt + Shift (Mac), and respond with Yes to the dialog asking if you really want to replace the preferences. If you aren't asked the question, you didn't hold down the keys quickly enough!

Did you add a Mailto: action to the send button in InDesign and did you export it as an Interactive PDF? And did you Reader enable the form so that users with only Acrobat Reader can enter data and store it?
