You can try the below steps and let us know if it works:
Open your PDF form in Acrobat.
Go to Prepare Form mode.
Name the fields as follows for reference:
"Yes_Checkbox"
"No_Checkbox"
"Explain_Field"
Right-click on the No_Checkbox, select Properties > Actions tab.
Under Select Trigger, choose Mouse Up.
Under Select Action, choose Run a JavaScript, then click Add.
Enter the following
if (this.getField("No_Checkbox").value === "Yes") { this.getField("Explain_Field").required = true; } else { this.getField("Explain_Field").required = false; }
Next, lock "Explain" Field When 'Yes' is Selected
Right-click the Yes_Checkbox, select Properties > Actions tab.
Under Select Trigger, choose Mouse Up.
Under Select Action, choose Run a JavaScript, then click Add.
Enter this
if (this.getField("Yes_Checkbox").value === "Yes") { this.getField("Explain_Field").readonly = true; this.getField("Explain_Field").value = ""; // Clears text if already entered } else { this.getField("Explain_Field").readonly = false; }
Note: This is a sample code that might or might not work for you. I am no expert with Scripting, and this is something I tried at my end.

When you used "Fix problems in PDF tagging structure" in Preflight, Acrobat likely attempted to automatically generate missing alternate text for annotations (including hyperlinks). This process can sometimes introduce extra text in the tooltips of hyperlinks because Acrobat may extract surrounding text, URL data, or metadata to fill in the missing alternate description.
To fix this, you can try the below steps:
Open Acrobat Pro and go to Accessibility > Reading Order tool.
Find the hyperlink and right-click on it.
Choose Edit Alternate Text.
Remove unnecessary text and keep only relevant link descriptions.
Save the file and revalidate in PAC.

You can try the below steps:
Open your PDF in Acrobat.
Go to Tools > Print Production > Preflight.
Click on Fixups (wrench icon).
In the search bar, type "Remove document information".
Look for "Remove document information and metadata" and select it.
Click Apply and save the file.
After removing metadata, re-run the Preflight tool:
Go to: Tools > Print Production > Preflight.
Select PDF/X-6 compliance check and verify that the issue is resolved.

What you can do here is, click the checkbox to the left of the file name and remove from recent.
Once done, go over to Menu-> Help-> Repair Installation-> Restart.

You are within Prepare Form due to which you are not seeing the options.
Click the backwards arrow to the left of where "Prepare Form" is written. The options should appear there.

If multiple users need different levels of access, you can consider using Adobe Licensing Management to enforce Shared Device Licensing instead of user-based activation.
The other option would be to suppress Acrobat from upgrading to Pro. To do so,
1. Go to Registry Editor;
2. Go to: HKEY_LOCAL_MACHINE\SOFTWARE\Adobe\Acrobat Reader\DC\FeatureLockDown
3. Create a new DWORD (32-bit) Value: bDisableUpsell and set its value to 1.

To answer your question:
Yes! When you insert a new page into a PDF, headers and footers do not automatically carry over to the new page. Instead, the new page is treated as a separate from the existing header/footer settings.
You can try the steps below:
Go to Edit > Header & Footer > Add... .
Note the current settings (text, alignment, font, etc.).
Click "Cancel" (so you don’t overwrite existing headers yet).
Go to Edit > Header & Footer > Update... .
Click "Apply to all pages", including the newly inserted page.
This method reapplies the same header/footer across the entire document, including the new page.

Here are some of my comments which I believe could be causing the issue:
Word’s Export to PDF Misinterpreting Styles
Even though your Word document uses styles properly, sometimes the tags can be wrongly assigned due to Word’s XML structure.
Custom Styles in Word Not Mapping Correctly to PDF Tags
If you've customized the Heading 1, Heading 2, and Heading 3 styles, those may not be mapped to the expected tags.
Word’s Accessibility Checker Does Not Validate Tag Structure
Word’s built-in checker only ensures that headings exist, but it does not verify correct tag assignment in the final PDF.
That being said, if the document’s structure is completely broken, you can try re-autotagging it:
Steps:
Open Check for Accessibility > Autotag Document .
Acrobat will attempt to reassign the correct tags.
Check the Tags Panel to confirm if the headings are now properly structured.

If Acrobat is still checking for updates even when auto-update is disabled, you can prevent the Updater from running:
Open Terminal and run: sudo chmod -x /Library/Application\ Support/Adobe/Acrobat/Acrobat\ Updater/Acrobat\ https://adobe.ly/3DXIB82\ Updater
This makes the updater non-executable, preventing it from launching in the background.
Also, Creative Cloud Desktop App updates Acrobat to the latest planned update, while Acroabt through the standalone app will check for any optional updates too.

You can remove cache files by following the below steps:
Access the Library Folder:
Open Finder.
Click "Go" in the menu bar.
Hold down the Option key and click "Library".
Alternatively, you can click "Go to Folder" and type in ~/Library .
Locate the Cache Files:
Open the "~/Library/Application Support/Adobe/Common/Media Cache" folder.
Open the "~/Library/Caches/Adobe" folder.
delete the files within the folders.
Empty the Trash:
Right-click the Trash icon in the Dock and select "Empty Trash".
Also, you can stop Acrobat from creating auto-save files, which can lead to cache memory consumption by going to Acrobat> Preferences> Documents> uncheck "Automatically save document changes."

Have you tried resetting the default app for PDF files, and re-assigning Acrobat to it?
This could be a glitch in the initialization of the process which a simple reset-and-reassign can fix.
Also, you can stop this by going to Preferences-> General-> uncheck the checkbox saying "Show me messages when I launch Acrobat."
